What Each Section Captures

  • A (Assessment): your clinical interpretation, numbered diagnosis and differentials.

A Filled Example: Canine Wellness + Mild Otitis

Patient: 3yo Labrador "Cooper", annual + ear scratching 3 days.

S: Owner reports doing well at home. R ear scratching and head shaking 3 days. Diet: Royal Canin Adult. Heartworm/flea: Simparica Trio monthly.
O: BW 32 kg. BCS 5/9. DA2PP SQ R shoulder. Rabies 3yr SQ L hip. EENT: OD mild erythema, moderate brown ceruminous debris, malodorous. OS WNL. Cytology: 2+ Malassezia.
A: 1. Annual Wellness (Healthy). 2. Otitis Externa (Yeast).
