Cornerstone, ezyVet and AVImark Alternatives (2026 Guide)

Side-by-side comparison of a legacy veterinary PIMS interface and a modern cloud dashboard

Most U.S. small-animal clinics run one of three systems: Cornerstone, ezyVet or AVImark. Each has a different reason people leave it. Cornerstone and AVImark are server-based and show their age. ezyVet is modern and capable, but complex and expensive for smaller teams.

This guide covers all three in one place: why clinics switch, which cloud systems they actually move to, what to check about lab integration, and the migration problems specific to each. Jump to Cornerstone, ezyVet or AVImark.

The short list, at a glance

The same handful of cloud systems come up no matter which one you are leaving. Where they fit best:

  • ezyVet: deepest feature set and reporting; higher cost and a longer learning curve.
  • Provet Cloud: cloud-native and modular; a common pick for multi-doctor and multi-site practices.
  • Vetspire: modern interface and strong analytics; popular with growing groups.
  • Shepherd: lightweight, opinionated charting built by vets; best for 1 to 4 doctor practices.
  • Digitail: mobile-first with a strong client app.
  • Chartling: AI scribe that drafts the SOAP from the exam conversation, plus scheduling, invoicing, controlled-substance logs and a client portal. It can also run next to your current system, so you can cut charting time before you migrate.

Cornerstone alternatives

Cornerstone has been the default vet PIMS for two decades, and it works. But the interface is dated, the server is a liability, and modern features such as online booking, AI scribes and integrated payments usually come from third parties.

Why clinics leave

  • Server maintenance, backups and the IT contract that comes with them.
  • Common actions take several clicks that a cloud system does in one.
  • Remote access for treatment-room laptops and after-hours charting is painful.
  • Upgrades are scheduled events; cloud vendors ship continuously.

Where they go

ezyVet

The closest feature-parity replacement. You won't outgrow it, but expect a bigger budget and more training.

Provet Cloud

Simpler than ezyVet with modular pricing.

Vetspire

Modern interface, strong analytics, integrations with major reference labs.

Shepherd

A good fit if your main complaint is charting speed in a small practice.

Chartling

Run the AI scribe next to Cornerstone today and copy the finished SOAP into the chart in one click, or move the whole clinic over later.

The IDEXX question

The strongest reason to stay on Cornerstone is its tight IDEXX integration. Cloud alternatives integrate with IDEXX too, but depth varies. Ask your IDEXX rep and each vendor specifically whether in-clinic analyzer results (Catalyst, ProCyte) post to the chart automatically, not just whether you can place reference lab orders.

Migration gotchas

Cornerstone exports work but are messy. Product codes, vaccine histories and reminder rules rarely map cleanly, so plan time to clean them up before cutover.

ezyVet alternatives

ezyVet is one of the most capable systems on the market. Clinics leave it not because it is old, but because they pay for depth their team doesn't use.

Why clinics leave

  • Per-user pricing adds up quickly in mid-size teams.
  • Implementation is long and takes real staff time.
  • New front-desk hires need weeks to become productive.
  • Custom templates and configuration often need paid help.

Where they go

Provet Cloud

The closest feature match with a cleaner interface.

Vetspire

Strong reporting and faster onboarding.

Shepherd

For small practices that find ezyVet overkill.

Digitail

If client engagement through an app is the priority.

Chartling

Scheduling, invoicing, controlled-substance logs, a client portal and an AI scribe in one system, built to remove documentation rather than digitize it.

Migration gotchas

ezyVet exports cleanly. The slow part is mapping the custom fields, templates and workflows your team built over the years. Inventory those first, and decide which ones you actually still need.

AVImark alternatives

AVImark has run U.S. small-animal clinics for about thirty years. Development has slowed, the interface shows its age, and the server-based setup limits what you can add.

Why clinics leave

  • Modern features arrive late, if at all.
  • Server maintenance and IT costs.
  • Remote access through RDP or Citrix workarounds that staff dislike.
  • Anything beyond basic reports means exporting to a spreadsheet.

Where they go

Shepherd

Often chosen by small practices coming off legacy systems.

Provet Cloud

Scales to multi-doctor and multi-site, with migration support.

ezyVet

The everything option, if budget and training time allow.

Vetspire

A fit for growing groups.

Chartling

AI scribe plus the day-to-day clinic tools, or run the scribe next to AVImark first.

Migration gotchas

  • Treatment codes. AVImark's coding is non-standard; budget time to remap.
  • Linked items. Multi-line treatments such as surgery packs need rebuilding.
  • Attachments. Documents are stored as file paths, and migration tools may not bring the files themselves.
  • Vaccine reminders. Audit that reminder rules carried over; this is where clinics most often lose data.

A migration plan that works for any of the three

  1. Pick a slow month. Never cut over during your busiest season.
  2. Budget 6 to 10 weeks from contract to go-live for a typical small or mid-size practice, plus staff training time.
  3. Clean data before export: merge duplicate clients and patients, retire unused codes, and fix reminder rules in the old system first.
  4. Test-migrate a sample of 20 to 30 real patients and check histories, vaccines, balances and attachments by hand.
  5. Run both systems in parallel for about two weeks.
  6. Keep the old system read-only for at least 90 days for chart lookup.
  7. Ask about migration fees. Vendors often discount or rebate them, especially when you are leaving a competitor.

Questions to ask every vendor in the demo

  • Show me in-clinic analyzer results posting to a chart, live.
  • What exactly comes over from my current system, and what doesn't (attachments, reminders, estimates, balances)?
  • What does pricing look like when we add a doctor or a second location?
  • How long until a new receptionist can check a patient in alone?
  • Can I export all my data, in a usable format, if I leave?

Not ready to switch? Add the AI scribe first.
